My First Visit

We look forward to welcoming you to our practice! For your first appointment, we ask that you arrive 15 mins early. You’ll be greeted by one of our friendly team members, who will assist you in completing your new patient paperwork at our practice. For your first visit, you can expect a comprehensive hygiene exam, dental x-rays, and a one-on-one consultation with Dr. Castillo to help create the smile of your dreams.

You first visit will include a consultation to discuss your concerns, review your medical history, and provide both diagnosis and treatment options. If you require surgery, our team will help you schedule your next appointment in our office.

Referral information 
Insurance card or any completed insurance forms

Please Note: Patients under the age of 18 must be accompanied by a parent or guardian in order to be seen.
